Percival accomplished the feat without a miss in a 29-16 win at Philadelphia on Oct. 20, 1968.

In seven seasons with the Bears from 1967-73, Percival connected on 99 of 182 field goal attempts.

He currently ranks fifth on the franchise's all-time scoring list with 456 points behind Butler (1,116), Walter Payton (750), Bob Thomas (629) and George Blanda (541).
